Medicare Only Patients
Medicare Patient with this card only is billed the Part B Deductible $257 (for 2025) then 20% of all Medicare Approved Charges unlimited except for annual checkups and preventative services
Medicare Patient with a Medicare Supplement Plan G card is billed the Part B Deductible but NOT the 20% of all Medicare Approved Charges. Other Medicare Supplement Plan types may pay more/less
Medicare Patient with a $0 Premium Medicare Advantage card will normally receive a $0 co-pay for virtually all Medicare Approved PCP services this office provides including (office charges, x-ray, lab, preventative) many plans offer benefits such as Over the counter items, Dental, Vision, Hearing, Gym membership
Medicare with Medicaid Patients
Medicare/Medicaid Patient with a QMB, QMB+, or MQMB card are generally NOT billed for any Medicare Approved Charges and may receive other healthcare related benefits
